This page explains how Dublin Ranch Dental treats information collected via this website. Our complete HIPAA Notice of Privacy Practices — covering protected health information (PHI) handled inside the practice — is provided at your first visit and is available on request by emailing dublinranchdental@gmail.com.
Information we collect on this website
- Voluntary submissions — name, phone, email, preferred appointment type, preferred time, and any optional notes you choose to share via our appointment-request or contact forms.
- Standard usage data — aggregated, non-identifying page-view information (URL, source, device type, anonymized IP). We do not use cross-site tracking pixels.
How we use the information
- To respond to appointment requests and patient inquiries.
- To improve the website and new-patient experience.
- We do not sell, rent, or share your personal information with any party except as needed to operate this website (see “Third-party processors” below) or as required by law.
Third-party processors
To deliver the appointment-request form to our practice inbox, we use FormSubmit.co, a forwarding service that transmits form submissions by email and does not retain submission contents beyond the time required to deliver them. FormSubmit’s privacy practices are described at formsubmit.co. We have selected this service because it forwards messages directly to our practice inbox without exposing our email address publicly and does not require third-party login or account creation.
For website hosting and general operations we use industry-standard cloud providers under written service agreements that prohibit secondary use of patient data. Where required by HIPAA, we maintain Business Associate Agreements with any vendor that may incidentally handle Protected Health Information.
Children’s privacy
We do not knowingly collect personal information from children under 13 through this website. Parents who book appointments for minor children remain the responsible custodian of that information.
Cookies
We use a small number of essential cookies for site functionality (form submission, accessibility preferences) and privacy-respecting first-party analytics. We do not use advertising or behavioral-tracking cookies. You may disable non-essential cookies in your browser at any time without losing functionality.
Your rights
Under California law (CCPA/CPRA) and other applicable regulations, you have the right to know what information we hold about you, to request its correction or deletion, and to opt out of any sale or sharing of personal information (we do neither). To exercise any of these rights, email dublinranchdental@gmail.com or call (925) 999-9088. We respond to verifiable consumer requests within 45 days. Note that dental records retained for clinical or legal purposes are subject to separate retention requirements under California Business and Professions Code §1680 and HIPAA.
Data security
This website is served over HTTPS with modern TLS encryption. Patient health information is not transmitted through public form fields; any sensitive clinical detail should be communicated by phone or in person.
